It's basically the same first few parts as the Snippet submission form (asking for a title if you have it, character/pairings, fandom...if it's original just put Original Work...warnings if needed and a brief description of the work to entice someone to claim it). The actual submission page is a little different. Please upload your thumbnail to <a href="https://postimages.org/" target="_blank">PostImages.org</a> or another image saving server (just no Photoshop, please...they're doing their users dirty right now) and link it in Thumbnail #1.
